How it works
A simpler agreement between you and your doctor.
You pay the practice directly each month. Because we are not billing insurance for routine primary care, visits can be longer, scheduling is easier, and the patient panel stays intentionally small.
Your insurance, health plan, or other coverage continues to be used for care outside the membership — hospitals, emergencies, specialists, imaging, and prescriptions.

Placeholder terms
- This Direct Primary Care Membership Agreement is a placeholder. Final legal language will be supplied by the practice before launch.
- Membership is a monthly fee paid directly to the practice for the defined primary-care services described in the membership terms. Membership is not health insurance.
- The member agrees to maintain appropriate coverage for services outside the membership, including emergency care, hospitalization, specialty care, imaging, and prescription medications.
- Membership is month-to-month and may be cancelled by either party with 30 days' written notice. Fees already paid for the current month are non-refundable.
- Medical records and protected health information are maintained in the practice's HIPAA-compliant medical record system, not on this website.
